“A health team is a group of persons who work together to
promote better health in the community”.
The health team members function according to the rules
laid down by the Ministry of Health and Family Welfare, Govt of India in
consonance with their policies.
A team of health personnel together can provide better
health services than when they are functioning alone.
A team is defined as group of persons with different levels
of knowledge, abilities and personalities who must complement each other and
who share a common goal.

Members of Health Team
Role of Public Health Nurse
- Providing health care (Preventive, Promotive, Curative, and Rehabilitative) to all individuals, families and community in the respective field.
- Planning RCH including family planning programmes with health workers.
- Training, supervising and guiding the health workers in the delivery of health care services to community.
- Follow-up services in school health/family health/family welfare programmes.
- Assisting in implementation of national health programme.
- Carrying out supervisory functions of the home visits in the area covered by the health workers.
- Co-ordinating the work of male and female health workers and other personnel.
- Arranging in identification of the areas requiring study/research in the community health nursing.
- Arranging educational programmes for the community.
Role of District Public Health Nursing Officer
- Supervising the family planning activities in the district level.
- Work as an advisor on all matters related to nursing services and education at the district.
- Assisting in determination of the philosophy and objectives of the community health programmes.
- Providing leadership in the planning of total health care to all individuals, families and community in her assigned area.
- Assisting in development of the standard of the health care.
- Assisting in the preparation of budget for the community health department.
- Promote the utilization of allotted budget.
- Supervision and guidance of PHN/LHV/ANM and other workers.
- Organizing educational programme at district level and in the community.
- Supervision of reports and records prepared by health workers working under her jurisdiction.

Role of Leady Health Visitors (LHV)
- Lady health visitor is expected to cover a population of 20,000 to 30,000 in which there are six subcentres.
- Supervise and guide the health worker ANM, Dais, and ASHA in the delivery of health care services to the community.
- Strengthen the knowledge and skills of the ANM.
- Help the ANM in planning and organising her programme of activities.
- Visit each subcentre at least once a week on a fixed day to observe and guide the ANM in her day to day activities.
- Assess the progress of work of the ANM and submit an assessment report to the medical officer of the primary health centre.
- Helps the health workers to work as part of the health team.
- Attend staff meetings at the primary health centre.
- Monitor the supply of drugs and equipment's at sub centre level.
- Ensure that the sub centre is kept clean and is properly maintained.
- Ensure that the ANM maintains her general kit and midwifery kit in the proper way.
- Scrutinize the maintenance of record by the health worker female and guide her in their proper maintenance.
- Review reports received from ANM, consolidate them and submit monthly reports to the medical officer of primary health centre.
- Organize and conduct training for Dais with the assistance of the Health Worker Female.
- Conduct weekly MCH clinics at each subcentre with the assistant of the ANM and ASHA.
Role and Responsibility of Axillary Nurse Midwife (ANM)
- Register and provide care to pregnant women throughout the period of pregnancy.
- Ensure all pregnant women take antenatal check-up.
- Test pregnant women urine for protein, BP, Hb, sugar etc.
- Promote the women for institutional delivery.
- Conduct delivery in her area.
- Identify the high-risk cases and refer them.
- Family planning, nutrition, immunization, and control of communicable disease, personal and environmental hygiene.
- Supervise and coordinate with ASHA.
- Perform at list one visit after delivery of a baby with ASHA.
- Maintain record at subcentre level.
Role of Accredited Social Health Activist (ASHA)
- Create awareness and provide information to the community people on nutrition, basic sanitation, and healthy living.
- Counsel the women on birth preparedness, motivate women to give birth in hospital, and importance of safe deliveries.
- Aware the mother about breastfeeding and complementary feeding.
- Bringing children to immunization clinics.
- Aware the eligible couple for family planning methods.
- Aware people about prevention of common infections including reproductive tract infections.
- Aware the community people about health, and health related services available at the Anganwadi, Subcentre, Primary health centre.
- Helps in development of comprehensive village health plan.
- She will inform about the births and deaths in her village and unusual health problems in the community to subcentre, and primary health centre.
- ASHA’s are also meant to serve as a key communication mechanism between health care system and rural population.
